Report Form 
At the START Institute you spent some time with your team
to begin planning how to most effectively replicate the institute
training modules in your state. Now we'd like to hear from you!
What have you done?
Please remember, your team is committed to carrying out a
minimum of eight days of training using START materials and procedures
modeled at the START Institute. Your audience may include university
education faculty, arts and sciences faculty, university administrators,
preservice education students, K-12 teachers, library/media specialists
and administrators. Training should be completed by July 2002.
Please complete this form for each replication
activity. List all team members who worked with you for each
activity.
Note: Don't forget to submit the Evaluation Summary for each
training activity. The Workshop Evaluation and Evaluation Summary
forms can be found at the end of each module in your START Trainer
Guide.
